Portfolio Requirements: Slides and/or digital media
Slides: An annotated slide list with the applicant's name must accompany the portfolio. The list should include the following information for each work: slide number, title, size, medium, and date of work.

• 20 high-quality slides
  
• Slides must be submitted in a clear plastic slide sheet
  

Digital media: An annotated list with the applicant's name must accompany the portfolio. The list should include the following information for each work: original format and length, title, size, portfolio medium, and date of work.
Portfolio should be submitted in only one of the following formats:

• VHS videotape no longer than 10 minutes queued to portion to be viewed
  
• DVD (NTSC only)
  
• Macintosh format CD (such as self-contained slide presentations on Powerpoint or Flash using a U.S. operating system)
  
• URL
